The HealthyLife HRA team is committed to ensuring the privacy and accuracy of your personal information. At present we do not save any personally identifying information. We will not actively share any information about your usage of the web site or your responses gathered from our servers. However, we may be compelled by law to release information gathered from our Web servers (including the summary server log information, e-mails sent to the web site, and information collected from Web-based forms) in some cases.
We are currently working to meeting HIPAA compliance standards so that health care organizations using the software will be able to save identifying information in order to serve their patients.
We are committed to protecting the privacy of all visitors to our web site. The web site contains links to hundreds of external Web sites. We are not responsible for the privacy practices or the content of the external Web sites to which we link.
Information We Gather
Our Web servers store your HRA responses in a secure database and generate temporary logs that contain the following information:
- Internet address of computer being used
- Web pages requested
- Referring web page
- Browser used
- Date and time
The data is used in aggregate to tune the Web site for its efficiency and is not ordinarily associated with specific individuals. Summary reports produced from the logs help web publishers determine what Web browsers and pages are most popular. Some portions of our site may also use cookies to retain user preference information.
Your answers to HRA questions and the results of your report are confidential and individual responses will not be shared with other parties unless required by law. Aggregate data from surveys may be shared with external third parties.
